This review aims to analyze the pharmacokinetic (PK) and pharmacodynamic (PD) characteristics of ACTs, with emphasis on absorption, metabolism, excretion, efficacy, and resistance dynamics, especially in vulnerable populations such as children and pregnant women.</p> <p><strong>Methodology: </strong>A narrative review approach was used. Relevant peer-reviewed literature published between January 1990 and April 2024 was identified through databases including PubMed, ScienceDirect, Scopus, and Google Scholar. Search terms included combinations of "artemisinin," "ACT," "pharmacokinetics," "pharmacodynamics," "antimalarial resistance," and related terms. Studies were included if they addressed ACT pharmacology or clinical outcomes in the context of malaria treatment.</p> <p><strong>Results: </strong>Artemisinin derivatives, including artesunate, artemether, and dihydroartemisinin, exhibit rapid parasite clearance due to short half-lives, but require combination with partner drugs such as lumefantrine or piperaquine for prolonged efficacy. Factors such as CYP450-mediated metabolism, drug bioavailability, genetic polymorphisms, and altered pharmacokinetics in special populations significantly influence treatment outcomes. Population PK/PD modeling and fixed-dose combinations help optimize dosing and minimize resistance. However, challenges persist in overcoming regional resistance patterns and ensuring accessibility in resource-limited settings.</p> <p><strong>Conclusion: </strong>Optimizing ACTs demands an integrative understanding of drug behavior, individualized dosing strategies, and continued pharmacological research.
